Shipping
costs are based on the package weight and
dimensional weight of your order and the
shipping method you choose. All orders are
shipped via UPS (United Parcel Service).
You may select Normal Ground Delivery (3-5
days), Third Day Select (3-day maximum),
Second Day Air (2-day maximum) and Next-Day
Air (morning or afternoon delivery - Saturday
delivery if it is available for your area).

| Please be aware that
you may have to pay local customs and duties in
order for your package to be released to you by
your local customs agent. We have no way of determining
these costs as they vary from country to country.
You may have to pay import duties and or other charges
(including GST/VAT) on your purchases. These are
assessed and charged by your government's customs
authorities, not by us, and those charges are not
included in the prices shown. Please note, it is
your responsibility to know the laws of your country,
state and / or municipality. We will NOT refund
money for the confiscation of items or packages
shipped to countries where customs policies do not
allow for entry of such products. However, we will
refund money for products whose packages have been
returned to us from customs |

| If an item you ordered
is out of stock, it will be back-ordered automatically
(domestic orders only). For international orders,
we will e-mail you of the missing items to confirm
the balance of your order. We try our best to keep
adequate inventory levels in order to ensure fast
and dependable shipments. However, on the rare occasions
when an inventory supply of a particular health
test is depleted, we will e-mail you within two
business days to inform you that the product has
been back-ordered. |

Our goal is to please our customer and provide the

Customer satisfaction is always our priority. Each
order is accompanied by very detailed, yet simple
instructions to assure satisfactory results from
your product. It is important that you carefully
follow the simple instructions enclosed with your
order to ensure the integrity of each item purchased.
We use the greatest possible care in packing our
shipments. Because we want you to be pleased with
your order, all drug and alcohol test kits carry
a 14-day return guarantee on any unused, unopened
test kits. If you are not satisfied with your purchase
in the first 14-days, simply follow the return steps
below and send unused items in their original packaging
and original invoice back to our warehouse. A full
refund including shipping charges will be issued
within 21 days if the return is due to our error,
or if any fall prevention products arrive damaged,
defective or tampered with. An e-mail confirmation
will be sent to you. Notification by customer must
be made within 72 hours of delivery in order to
receive full credit for any product damaged or any
loss occurring in transit.
Return Steps:
1. Returns will be accepted up to 14
days from the purchase date for any unused and saleable
products, and must be accompanied by the original
invoice andoriginal packaging;
2. Refunds, exchanges, or credits will
not be given on merchandise that have been used,
tampered, opened or defaced in any manner;
3. Refunds will be made in the same form
as original payment. (If payment was with a check,
please allow 14 business days. Shipping and handling
charges are non-refundable;
4. Mail the product(s) to: Horizon
Medical Technologies at the address indicated
at the top of this page;
5. Returns are subject to a 15% restocking
fee. |

Terms |
| Terms are Net 30 days
with approved credit and are payable in U.S. currency.
Accounts not paid within 30 days are subject to
applicable interest (1.5% per month as allowed by
law). A $25.00 fee is charged for any returned checks.
We may extend credit terms to government agencies,
school districts, and other established institutions
like hospitals and medical clinics. Please e-mail

up an account or make other arrangements. |
